CCS program costs
Families with CCS scholarships pay a portion of the child care cost. This may be called a copayment or parent share of cost. Income limits for CCS eligibility, parent share of cost sliding fee scale, and the parent share of cost calculator are updated at least once per year. View the current documents below.
Parent Rights & Responsibilities form
When applying for CCS, families have rights and also must agree to CCS program rules and requirements. Families' rights and responsibilities and the CCS program rules and requirements are described in the Parent Rights & Responsibilities form.
Employment/Income Verification form
The Employment/Income Verification form is used to document a family’s income and work hours when pay stubs are not available. Pay stubs may not be available if a family member has recently started new employment, has pay documents that don't show both gross pay and hours worked, or is paid in cash.

Parent Change Request forms
Parent Change Request forms are filled out when there has been a change in your household circumstances.
There are two different Parent Change Request forms - one if you're on the waitlist and the other is after you submit your standard application.
All reasons for a change are listed on the forms but the most common include household changes, the family moved, contact information changes, change in income, name change, and a provider transfer. Please review the forms for a list of all changes and the changes that must be made within 14 calendar days.
Parent Provider Selection form
Fill out the Parent Provider Selection form when you have chosen your child care provider.
Certification for Inclusion Assistance Rate form
CCS providers can be paid a higher rate to care for your child with special needs or disabilities. The higher rate helps providers pay for special equipment, materials and supplies, and extra staff to provide the care your child needs.
Parents must start the process by downloading and filling out the Certification for Inclusion Assistance Rate from. The form has parts for the parent or guardian, the child care provider, and a qualified professional from the local Workforce Solutions office to fill out. After you download the form, complete the parent or guardian sections and contact your local Workforce Solutions office to start the process.

All families enrolled in CCS must track their child's attendance. This document has tips for families tracking attendance. This document is not a form to be filled out.
